When Therapy Gets Stuck: Practical DBT Strategies to Break Through Clinical Impasses

Even the most skilled clinicians encounter cases that seem to go in circles. Sessions become dominated by crises, emotional escalation, avoidance, reassurance-seeking, shutdown, or repeated patterns that prevent meaningful progress. Despite best efforts, therapy can begin to feel stuck, frustrating both clinicians and clients.

This engaging and highly practical training provides clinicians with a DBT-informed framework for understanding why therapy stalls and what to do when it does. Designed for mental health professionals who want effective interventions without needing extensive DBT specialization, this workshop focuses on identifying and interrupting the behavioral and interpersonal patterns that can unintentionally maintain treatment impasses.

Participants will learn how to distinguish situations that require stabilization from those that are ready for problem-solving or behavioral intervention, while examining how client behaviors, therapist responses, and treatment structures can inadvertently reinforce ongoing difficulties. Through behavior chain analysis and solution analysis, clinicians will develop a deeper understanding of behavioral function, reinforcement processes, and strategic intervention points that promote real change.

Rather than teaching a collection of isolated coping skills, this training equips clinicians with a practical decision-making framework for selecting DBT interventions based on clinical context, emotional intensity, behavioral function, and treatment goals. Through case examples, clinical demonstrations, interactive polls, and guided application exercises, participants will leave with concrete strategies they can immediately integrate into their clinical work.

Whether you are working with chronic crises, therapy-interfering behaviors, emotional dysregulation, or clients who seem unable to move forward, this training offers practical tools to help therapy regain momentum and create more effective treatment outcomes.
